SQL Report query

Hi All,

I am trying to discover what all routers are available in the environment. I have the router asset serial number.

I and try to get the columns as 

(Router) Asset Serial 
Host name
IP Address
Server name
Model 
Place of Installation
Date of Installation

I am not able to get the details. Anybody has tried this?
